Only horizontal eye movements will be tested. Voluntary or reflexive (oculocephalic) eye movements will be scored, but caloric testing is not done. If the patient has a conjugate deviation of the eyes that can be overcome by voluntary or reflexive activity, the score will be 1. If a patient has an isolated peripheral nerve paresis (CN III, IV or VI), score a 1. Gaze is testable in all aphasic patients. Patients with ocular trauma, bandages, pre-existing blindness, or other disorder of visual acuity or fields should be tested with reflexive movements, and a choice made by the investigator. Establishing eye contact and then moving about the patient from side to side will occasionally clarify the presence of a partial gaze palsy.
Testují se jen horizontální pohyby bulbů (volní či vyvolané okulocefalické reflexy). Neprovádí se kalorické testování.
- Izolovaná paresa okohybného nervu či deviace, kterou lze překonat okulocefalickým manévrem jsou hodnoceny 1.
- Deviace bulbů, kterou nelze překonat OC manévry, hodnotíme 2.
U afatických či amentních pacientů je vhodné navodit oční kontakt a pak se pohybovat kolem lůžka a sledovat reakce pacienta. V tomto bodě platí výjimka z pravidla o hodnocení první odpovědi a zákazu dopomoci. Hodnotí se nejlepší odpověď.

A great deal of information about comprehension will be obtained during the preceding sections of the examination. For this scale item, the patient is asked to describe what is happening in the attached picture, to name the items on the attached naming sheet and to read from the attached list of sentences. Comprehension is judged from responses here, as well as to all of the commands in the preceding general neurological exam. If visual loss interferes with the tests, ask the patient to identify objects placed in the hand, repeat, and produce speech. The intubated patient should be asked to write. The patient in a coma (item 1a=3) will automatically score 3 on this item. The examiner must choose a score for the patient with stupor or limited cooperation, but a score of 3 should be used only if the patient is mute and follows no one-step commands.
Mnoho informací o řečových funkcích získáme již během předchozího vyšetřování. Řeč vyšetřujeme tím, že pacient popisuje, co se děje na přiloženém obrázku, dále pojmenovává předměty a čte věty na dalších dvou přiložených listech. Žádejte, aby pacient přečetl všechny věty a pojmenoval všechny předměty. V případě, že porucha zraku znemožňuje toto vyšetření, nechte pacienta pojmenovat předměty vložené do ruky, nechte jej opakovat slova a spontánně produkovat řeč. Intubovaní pacienti mohou psát.
Pacient v komatu je skórován 3. Vyšetřující se musí rozhodnout, jak skórovat pacienta ve stuporu či s omezenou spoluprácí, ale 3 dostává jen pacient zcela bez slovní produkce, který nesplní žádný z vyšetřovaných úkolů.
Pozn. přiložené listy papíru mají potvrdit informace získané již z předešlého vyšetření. Přesto je nutné toto vyšetření provést tak, jak je předepsáno. Lehká afázie dostává skóre 1. Skóre 2 dostává pacient, který nevyjmenuje či nepřečte více než 2/3 předmětů a vět.

Pacient je požádán o otevření a zavření očí a potom o sevření a otevření neparetické ruky. Úkoly lze modifikovat dle možností pacienta (např. v případě kvadruparesy) a použít jiný úkol. Opět zaznamenáváme první pokus pacienta. Úkol je možno pantomimou předvést. Dle situace můžeme měnit pořadí příkazů a hodnotí se pouze druhá odpověď. Např. v případě, že má zavřené oči, požádáme ho, aby oči nejprve otevřel a potom zavřel a hodnotí se pouze jejich zavření.
The patient is asked to open and close the eyes and then to grip and release the non-paretic hand. Substitute another one step command if the hands cannot be used. Credit is given if an unequivocal attempt is made but not completed due to weakness. If the patient does not respond to command, the task should be demonstrated to him or her (pantomime), and the result scored (i.e., follows none, one or two commands). The investigator must choose a response if a full evaluation is prevented by such obstacles as an endotracheal tube, language barrier, orotracheal trauma/bandages. A 3 is scored only if the patient makes no movement (other than reflexive posturing) in response to noxious stimulation.
Vyšetření provádíme na základě předchozího kontaktu s pacientem, odebírání anamnesy, atd. Hodnotíme za všech okolností (včetně OTI, orotracheální poranění, apod.). Skóre 3 hodnotíme pouze, pokud pacient reaguje pouze na bolestivé stimuly a to reflexně či nereaguje vůbec. Tážeme se pacienta 2-3 otázkami na okolnosti přijetí do nemocnice, dále se ptáme na anamnézu, předchozí medikaci.

If patient is thought to be normal, an adequate sample of speech must be obtained by asking patient to read or repeat words from the attached list. If the patient has severe aphasia, the clarity of articulation of spontaneous speech can be rated. Only if the patient is intubated or has other physical barriers to producing speech, the examiner should record the score as untestable (UN), and clearly write an explanation for this choice. Do not tell the patient why he or she is being tested.
Je možné, že lehká dysartrie může být opomenuta při předchozím rozhovoru. Proto k testování použijeme přiložený list papíru, z kterého pacient přečte předepsaná slova.
Skóre 1 dostává pacient, který je dysartrický, ale kterému je rozumět.
Skóre 2 dostává pacient, kterému pro dysartrii není rozumět nebo který je mutistický.
Afatičtí pacienti a pacienti, kteří nemohou či neumí číst, mohou být skórováni na základě jejich spontánní slovní produkce nebo opakování lékařem nahlas přečtených slov. Jen pokud je pacient intubovaný, či je přítomna jiná fyzická překážka k produkci řeči, je skórován 9 a vyšetřující musí jasně definovat příčinu. Nesdělujte pacientovi, za jakým účelem je testován.

Dostatečné informace k rozpoznání neglect syndromu mohou být získány během provedeného předchozího vyšetření. K testování použijeme simultánní stimulaci zrakovou a sensitivní, popř. jinou kvalitou.
V případě izolované poruchy nebo anosognoze skórujeme 1, v případě extinkce pro více než 1 kvalitu, skórujeme 2. Pokud má pacient hemianopsii a jinak nic nesvědčí pro neglect (např. testování kožní citlivosti), skórujeme 0. Podobně pokud při afázii reaguje na obě strany, skórujeme 0.
Tento bod skórujeme jen v případě jasně prokazatelného deficitu a proto nemůže být netestovatelný.
Sufficient information to identify neglect may be obtained during the prior testing. If the patient has a severe visual loss preventing visual double simultaneous stimulation, and the cutaneous stimuli are normal, the score is normal. If the patient has aphasia but does appear to attend to both sides, the score is normal. The presence of visual spatial neglect or anosagnosia may also be taken as evidence of abnormality. Since the abnormality is scored only if present, the item is never untestable.

Ask – or use pantomime to encourage – the patient to show teeth or raise eyebrows and close eyes. Score symmetry of grimace in response to noxious stimuli in the poorly responsive or non-comprehending patient. If facial trauma/bandages, orotracheal tube, tape or other physical barriers obscure the face, these should be removed to the extent possible.

Cílem je diagnostikovat jednostrannou cerebelární lezi. Pacient má otevřené oči, v případě poruchy zorného vyšetřujeme v intaktním zorném poli. Testujeme pokus prst-nos-prst a koleno-pata. Vyšetření je možno pacientovi předvést.
- Ataxii hodnotíme jen pokud je přítomna, a to disproporčně k přítomné parese. U afatických či plegických pacientů hodnotíme 0.
- V případě amputace, ankylózy apod. hodnotíme 9 (uvedeme důvod).
- Pokud má pacient paresu a lehkou ataxii a nelze určit, že ataxie není proporční k parese, hodnotíme 0.
This item is aimed at finding evidence of a unilateral cerebellar lesion. Test with eyes open. In case of visual defect, ensure testing is done in intact visual field. The finger-nose-finger and heel-shin tests are performed on both sides, and ataxia is scored only if present out of proportion to weakness. Ataxia is absent in the patient who cannot understand or is paralyzed. Only in the case of amputation or joint fusion, the examiner should record the score as untestable (UN), and clearly write the explanation for this choice. In case of blindness, test by having the patient touch nose from extended arm position.

NIH Stroke Scale je standardizované neurologické vyšetření sloužící k popsání deficitu u pacientů s iktem. Cílem této škály je, aby různí vyšetřující hodnotili pacienty obdobně a výsledky tak byly srovnatelné. Nejlepší způsob testování je diskutabilní a dle této škály může být odlišný od klinické praxe. Pacient tak může být vyšetřen dvěma způsoby za cenu vysoké reprodukovatelnosti výsledků.
Zde jsou 4 základní principy hodnocení NIHSS:
1. Vždy hodnotíme první odpověď. Znamená to, že pokud např. pacient na otázku týkající se jeho věku odpoví nejprve špatně a potom se opraví, je nutno hodnotit odpověď jako nesprávnou. Toto je zásadní, protože jinak není možné zohlednit všechny způsoby verbální a nonverbální nápovědy a zajistit reprodukovatelnost.
2. Nikdy pacientovi při vyšetření nepomáháme a neradíme, pokud není v instrukcích výslovně uvedeno. Při vyšetření tak neškálujeme jeho nejlepší výkon.
3. Některé položky jsou hodnoceny, jen pokud jsou přítomny. Např. ataxie je u plegického pacienta hodnocena jako nepítomna.
4. Hodnotíme jen to, co pacient v danou chvíli dokáže, ne to co si myslíme, že by dokázat mohl. Skóruje se i starší deficit s výjimkou poruchy sense.
Skóre by mělo být zapsáno ihned po ukončení vyšetření.

Ptáme se na věk pacienta a jaký je nyní měsíc v roce. Počítá se pouze první a pouze zcela správná odpověď. Vyloučena je verbální či nonverbální nápověda nebo výběr z nabídnutých možností. Pacienti, kteří nemohou mluvit, mohou odpověď napsat.

The patient is asked the month and his/her age. The answer must be correct - there is no partial credit for being close. Aphasic and stuporous patients who do not comprehend the questions will score 2. Patients unable to speak because of endotracheal intubation, orotracheal trauma, severe dysarthria from any cause, language barrier, or any other problem not secondary to aphasia are given a 1. It is important that only the initial answer be graded and that the examiner not "help" the patient with verbal or non-verbal cues.

Sensation or grimace to pinprick when tested, or withdrawal from noxious stimulus in the obtunded or aphasic patient. Only sensory loss attributed to stroke is scored as abnormal and the examiner should test as many body areas (arms [not hands], legs, trunk, face) as needed to accurately check for hemisensory loss. A score of 2, “severe or total sensory loss,” should only be given when a severe or total loss of sensation can be clearly demonstrated. Stuporous and aphasic patients will, therefore, probably score 1 or 0. The patient with brainstem stroke who has bilateral loss of sensation is scored 2. If the patient does not respond and is quadriplegic, score 2. Patients in a coma (item 1a=3) are automatically given a 2 on this item.
U pacienta posuzujeme grimasu či kvantitu vjemu při bodnutí ostřejším předmětem (špendlík), či reakci na bolestivý stimulus u pacienta utlumeného. Hodnotí se jen postižení senzitivity vzniklé v důsledku mozkové příhody a testujeme dostatečné množství partií k validnímu posouzení rozsahu postižení pozor na testování aker vzhledem k možnosti senzitivní neuropatie a tudíž falešné pozitivitě.
Skórujeme 2 body při jasně prokazatelné těžké či úplné ztrátě citlivosti. Z toho dvodu budou pacienti stuporózní či afatičtí většinou skórováni 1 či 0.
Pacient s bilaterální ztrátou sense při kmenovém infarktu je skórován 2.
Pacient v kómatu je skórován 2.
Pozn. Netestujte akra končetin, protože výsledky mohou být zkresleny možnou polyneuropatií. Nevyšetřujte přes oděv.

Používáme simultánní stimulace pohybujícími se prsty (nebo jejich počítání) v obou zorných polích zvlášť v horním a dolním kvadrantu.
Skórujeme 1, pokud je jasná asymetrie, včetně kvadrantové hemianopsie.
Skórujeme 2 v případě kompletní hemianopsie.
Skórujeme 3 v případě slepoty (včetně kortikální). V případě enukleace či monokulární slepoty hodnotíme zorné pole na zdravém oku.
Zorné pole vyšetřujeme i simultánně a v případě extinkce hodnotíme 1 a odpověď použijeme též u bodu 11.
Visual fields (upper and lower quadrants) are tested by confrontation, using finger counting or visual threat, as appropriate. Patients may be encouraged, but if they look at the side of the moving fingers appropriately, this can be scored as normal. If there is unilateral blindness or enucleation, visual fields in the remaining eye are scored. Score 1 only if a clear-cut asymmetry, including quadrantanopia, is found. If patient is blind from any cause, score 3. Double simultaneous stimulation is performed at this point. If there is extinction, patient receives a 1, and the results are used to respond to item 11.
